This condition causes significant clinical problems and raises public health issues, affecting patients' quality of life and the management of associated complications. Methodology: To identify the factors associated with undernutrition in patients with decompensated cirrhosis, we conducted a cross-sectional analytical study at the Kamenge University Hospital. Data collection and analysis were carried out over a period of four months, from November 1, 2023 to February 29, 2024, and involved patients hospitalized in internal medicine with decompensated cirrhosis. To determine the factors associated with undernutrition, we used logistic regression; the measure of association used was the OR and its 95% CI; the significance threshold was set at 5%. Results: Of our participants, between 40-60 years, 64.71% were undernourished positive, 44.44% were undernourished negative; and over 60 years, 64.71% were undernourished positive, 35.29% were undernourished negative. Two factors were significant; „‟Anorexia„‟ (OR= 42.78;p value=0.012;CI=[2.26 808. 17]), in cases of decompensated cirrhosis, if you are anorexic, you are 42.78 times more likely to develop undernutrition than those who are not. and „psychological stress‟ (OR= 33. 47;p-value=0.038;CI=[1.2 923.52 ]), in the case of decompensated cirrhosis, if you have „psychological stress‟, your risk of developing undernutrition is 33.47 times higher than for those who do not. Conclusion: Undernutrition in patients with decompensated cirrhosis is a major clinical issue, as it can worsen their state of health and reduce the effectiveness of treatment. Assessment of undernutrition must take into account factors such as anorexia and psychological stress. Anorexia, which is common in these patients, is the result of metabolic changes, gastrointestinal disorders or the side-effects of treatment, leading to a loss of muscle mass and deterioration in immune function, thereby increasing the risk of complications.
